Requirements for a New U.S. Passport
What you will Need:
- Certified birth certificate or naturalization certificate or passport issued over 15 years ago.
- 2 passport size color photographs (2” x 2” with white background).
- Completed Form DS-11. This form must be completed online. Please visit the US State Dept for an on-line Form DS-11. Complete all of the relevant fields on-screen then click on "Continue". Follow the instructions and complete the form. You will finish by printing the required bar-coded application form. A completed application will have 3 bar codes: one on the left and one at the bottom of the 1st page and one at the bottom of the 2nd page. The passport office will not accept hand-written applications. Include a copy of your completed form on the outside of your sealed envelope referred to below.
- Government fees as follows: $160.00 for adults and $145.00 for minors under the age of 16 years.
- The fees are divided for ADULTS: $135 for “US Department of State” and $25.00 for the Post Office or the Clerk of Court.
- The fees are divided for MINORS: $120 for “US Department of State” and $25.00 for the Post Office or the Clerk of Court.
- Proof of departure date. Either a computer generated flight itinerary from the airline or a travel agency, copy of airline ticket, or a corporate letter stating that the U.S. departure will be by private aircraft.
- Reserve Processing Space & Print Letter of Authorization
- Please contact Embassy Document Services at 1-866-573-3797 and ask the representative to schedule an Urgent Reservation with the State Department.
- You will be directed to print 2 copies of a specific Letter of Authorization (LOA1, LOA2, LOA3 or LOA4) that can be selected below. The Letter of Authorization gives us permission to handle your passport application.
- NOTE that the 2 copies of Letter of Authorization must have original signatures of the passport applicant. No photo copies or faxes are permitted.
- Completed EDS Service Request Application Form Must be attached only to the outside of the sealed envelope.
- ALL DOCUMENTS MUST BE SIGNED IN BLUE INK ONLY.
- Passports are processed according to U.S. departure date.
What you need to do:
- Contact EDS to reserve a space for the Passport Office. (Item #6 above)
- Take items 1-6 (in the above list) to an authorized Post Office or Clerk of Court and ask them to endorse the documents and seal them in an envelope marked “TO BE OPENED BY PASSPORT OFFICE PERSONNEL ONLY”. We must present this sealed envelopeto the passport office within 5 working days from execution.
- DO NOT WRITE CHECKS until directed to do so by the clerk. ALL GOVERNMENT FEES MUST BE INSIDE THE SEALED ENVELOPE.
- You must present proof of identity such as a driver’s license (which has been issued for at least 6 months).
- MINORS – All minors must appear in person at the Post Office or the courthouse. Minors (under the age of 14) must be accompanied by both parents or legal guardian with acceptable proof of ID. If only one parent appears the other parent must sign and notarize a Statement of Consent Form DS-3053.
- DO NOT leave documents with the clerk.
- You must submit a copy of the form DS-11 and the EDS Service Request Application Form paper clipped to the outside of the “Sealed Envelope”. EDS cannot open the “Sealed Envelope”.
- Send the package to EDS by express delivery. EDS recommends Federal Express next day delivery.
